Operating and maintaining an electrical installation is business-critical and as electrical power is invisible, it is a risky activity.
This is why Safety, Maintenance and Operation Procedures and Authorization processes exist to secure processes and mitigate those risks. These activities are therefore highly regulated with corporate, global and local standards.
However, unexpected events can occur, with sometimes fatal consequences, and when roots cause analysis are conducted, human factors come first such as a lack of skills, lack of understanding of the electrical installation and difficulties having a clear detailed view of the whole electrical system.
In addition, issues can also come from lack of operational process carried-out and not updated, overloading, not up to date and confusing documentation, equipment mislabelling, difficulty to know the proper procedure vs mission or to identify the equipment itself due to their age or health status, and operators not being informed.
